Team Synergy

In the Hillandale Family of Communities, we use a proactive approach dubbed “Team Synergy” to make our three communities leaders in patient care reform.

What is Team Synergy?

The Team Synergy Initiative is a team of physicians, nurses, nursing assistants, nurse practitioners, therapists, pharmacists, dieticians, respiratory therapists and social workers who work to provide quality care and minimize the likelihood of our residents’ rehospitalization after an acute care hospitalization.

How does the team keep residents out of the hospital?

Our attention to the red flags that result in avoidable and unnecessary rehospitalization is key in keeping our long term care residents out of the hospital and on the path to recovery.

Our team of skilled nurses, physicians, therapists, pharmacists, dieticians, and social workers work together to promote quick intervention, early detection, and rapid treatment of any disease or illness that may cause rehospitalization.

This skilled team uses IV Therapy, Respiratory Therapy and other treatment modalities to help keep the healing process moving forward. Our supportive services for recovery include: Imaging, Pharmacy, Laboratory, Home Health Care, DME, and End-of-Life care.

What is the objective for Team Synergy?

The objective for the Team Synergy Initiative is the same as the patients’: to recover without spending more time or money at the hospital. Over the past 50 years, we’ve been a leading skilled nursing facility.

We recently redoubled our efforts at achieving significant improvements in the quality and safety of healthcare for those in our facilities. We want the transition from hospital to Hillandale and Hillandale to home to be safe, which is why we aim for high-quality care every step of the way.
